I have changed my name, what do I need to do?

You must tell us if you have changed your name. You can do this by filling in and sending us form AP1. In panel 4 under ‘Applications in priority order’, write ‘Change of name’.

There is no charge.

You must also send us evidence of the change, such as:

  • the original or an official copy of a marriage certificate or a civil partnership certificate, or
  • the original or a certified copy of a deed poll, or
  • the original statutory declaration, or
  • an original statement of truth.

If you want the original statutory declaration or statement of truth to be returned you also need to send in a certified copy. If you only send in an official copy we will keep it.

We will always return any original marriage or civil partnership certificates and deed polls.

You must also send in evidence of your identity when changing your name by:

  • deed poll, or
  • statement of truth, or
  • statutory declaration.

For advice on what evidence you need to send see Public Guide 22 – Keeping your name in the register up to date.

If you want to tell us about other changes, for example, you have also changed your address, you must send all applications to the Land Registry office that deals with property/land in your area.
